Jeremy - first off: congratulations on an incredibly thorough, well thought out and incredibly well executed project. Hats off & cheers to you!
Secondly: I'm assuming there's something inside that throttle cable to keep weather/moisture/critters from getting in to your cab? Wouldn't that be fun to have a baby squirrel come in that way & crawl up your leg while driving... "I'm AM being honest officer!! It was a baby squirrel that made me mash the pedal!! OHhhhh.... you've seen my youtube videos, have you...?"
one other resource may be a medium/ heavy duty truck shop... there is a place called great plains diesel that has a location near me that basically works on anything... including the cummins dodges. I have a difficult time believing that a place like that ( not dealer affiliated but cummins certified) would not be able to shed some light on the issue.
So are we having a Smarty FedEx'ed to us for a Saturday morning delivery?
There's no way in hell any of us want to wait until Monday...
I mean, you do need it anyway, right?
Negative. I gotta be sure I know that's it before I go and spend money that I really don't have right now. Smarty's run about $600. An overnight and Saturday morning delivery runs almost as much.

A squirrel? LOL. A couple things...that throttle under the hood has a cover that goes over it, and that big piece of brass is just a holder for an actual sheathed cable? If a squirrel can make it's way through a sheathed cable...
Maybe these pics will help. You see the little cable sticking out of the brass in the first picture, then the cable routed up into the firewall in the second one.

I'll check on that. Not sure there are any diesel places around here like that though. I've looked into it before.

It worked ok from what I could tell yesterday. I will definitely be messing with the shift file once I get it going right.
Jeremy, I'll give it up to you for tackling the electronics, that's what steered me away from the CR 5.9... In fact, it's steering me away from the PSD, I'm not going to mess with 7.3 in my '99. I had high hopes for it but oh well.

Thanks, Jet. The electronics I didn't think were all that tough, but I've been messing with electronic things and wiring in general since I was a kid. Now, I'm by no means a know it all with electronics, but I think the stuff for the conversion was pretty easy.
Update on the 2K rev ceiling. Two things in the works here.
Got word from Scott at Destroked that it's definitely a SKIM problem causing it. Waiting on a call back from Mid Atlantic Diesel to get the Smarty ordered.
Also ran by the Dodge dealership and talked to the diesel tech there. His opinion is that the SKIM code would cause a no start condition and it's got to be something else. He offered to check the codes with his Dodge specific scanner to see if there's something that AE may not be pulling. He told me to just bring the truck by and he'll ride around with me for a bit and look for any readings from sensors that might not be within spec. He's got several ideas of what it could be...MAP, ECT, EOT, TPS, etc... Needs to scan it with his scanner. He also said he can reset the TPS to make sure that WOT voltage really means WOT to the ECM.
Once I get my power steering hose fixed (hopefully today -- the hose dude was at the dentist this morning when I went by), I'm going to drive the truck to the Dodge dealership and have the tech check it over.
Now that's the ride I would like to take in it. To a Dodge stealership. Man, take a hidden video camera with you. Or better yet some kind of bug in their break room to hear what's said out of your earshot.
