Like Tom said, at least clean up the IPR if you don't want to replace it. If it were me, I would probably replace it with an OEM one, but that is just me and I like shiny parts.
Do us a favor though, post back with how the T-500 works out for you directly after install. There is a member on here that has been fighting one for a while, but we really don't know what the issue is.

I agree with Sous. You're already there and if it hasn't gone bad yet it will, and now you'll have a spare.
OTOH, I replace quite a few parts because I want to do as much preventative maintenance as possible.

With that many miles I would put in a new IPR. I did when I installed my T500. It probably had about 100K on it but I like to do preventive work when I have something torn down (as Sous and mecdac said). I'm not a fan of working blind so took the opportunity to replace the IPR when I had the perfect work area (HPOP out). I also am not a fan of rework and try to address as many things (current and future) as possible when there's a job with a lot of interference removal. I will admit, though, that I go way beyond what most others do (when time and $$$ permit).

I'd put in a new IPR or rebuild it, mine quit on me back in 2012 at about the same mileage. Killed myself trying to figure out what was preventing my truck from starting. I replaced the IPR with a new one, it fired right up, and has been running well ever since.
Same thing happened to El_Coqui, he couldn't figure out why his would randomly stall and not restart, we replaced the IPR and he was golden.

I am in the camp of "if i am in there and already replaccing one item and other items are coming out to complete the task then anything that comes out and is suspect I replace it with out question. This will allows me to keep the good still working parts as a spare incase something breaks. But that is just me and how I think.

I ran my ICP sensor until the engine started running rough. Pulled the plug and had oil in the connector and knew it was done for (250K-ish). Installed a new sensor and pigtail repair and was good to go. If you are concerned you can replace the sensor before it gives out and save yourself the pigtail repair also. The sensor is not IPR prices but definitely more than a CPS.
