Believe it or not, those plastic bushings don't really wear, not that its a bad idea to replace them while your in there. But, they usually aren't really responsible for play in the shifter. The brass that the shifter block is made from, where those plastic bushings ride, is what wears, along with the pivot pins. The brass is apparently softer then those bushings!
The short throw shifter from MidWest Trans uses steel instead of brass. I have my old shifter somewhere. If I get a chance, I'll post a pic of the wear in the brass.

Hmmm, I don't think my truck even has this second little boot. It was probably lost or removed due to the transmission being filled from the top for the past 16 years by the PO.
I will have to get this and the white cover when I purchase the MidWest short shift kit.
Not to spoil the surprise, but Carguy3j might be doing a write up on how to install the MidWest short shift kit and additional parts soon. At the very least he plans to provide the parts list which will make it onto the FTE with him being sited as the source and getting credit for the hard work...

The only problem is I didn't have time to take any pics during the job, so it won't be too exciting, but at least the info will be there.
As for the boot situation, All I saw on mine was the outer black boot, and that white rubber one inside it. I didn't see anything felt or foam. What I have seems to work just fine and keeps it quiet.

6mm Allen socket
Phillips screwdriver
Remove 2 torc bolts, then 4 Phillips panned screw. Lift outer boot off. Now, remove 6-6mm bolts, and remove assembly. With assy on workbench use flathead to remove ring to remove the damage boot. Now, install ring into new boot and into with finger pressure. From here, just reinstall everything
