Truck has these symptoms typically after an overnight sit. The conditions for this start were about 40 degrees, truck had sat for a few days. Typically if I was to plug it in for a few hours ahead of time it would fire up easier, however for this troubleshooting purpose I did not.
FICM is fine, REV-X added to oil, maybe ICP sensor bad? Any other suggestions? I know the injectors on this truck are old 181,000mi. The HPOP was replaced about 18 months ago.
how does it run after the truck is warmed up? have you checked the glow plugs? after the truck is warmed up does it fire up right away? you could try unplugging the icp (doesn't cost anything to try) but will throw a code. did you test the ficm koeo and koer at different rpms?

Once it fires it needs about 15-30 seconds and its running great. The rest of the day it starts up like a champ. It's always the first start of the day that it struggles.
