Additional work while replace head gaskets
Here is the million dollar question: If it was your truck with 170k miles, would you put lifters in while your in there for head gaskets? Have you seen enough to make you think lifers are wear items and are going to fail at X miles?

If you are going to pull the engine and mount it on a stand to do the work, it would be very prudent to address your cam and lifter issues (if any). As far as how often they fail, I personally can't give any intuitive or definitive answer on that unfortunately. I can only say that engines that have been neglected on oil changes, have high engine hour counts (relative to vehicle mileage) or abused in any way are definitely the ones that will MOST LIKELY have that issue.

I thought it was as easy as throwing in a set of lifters while the heads are off. I guess I could remove and inspect the ones that are accessible. Then, if any show excessive wear, replace them all.

Most of the ones that I have seen fail have been around the 350 to 400 K mark.
It only takes one to fail to trash the entire engine. Most of the time the rollers fall out and trash the LPOP and before you know it the whole engine is toast.
I think a lot of it has to do with maintainence of the oil, filter and how hard the engine has been worked during its life.
I now have around 250K on my 06 and if I have to go back inside the engine a new set of lifters are in order. I work mine pretty hard. About 60% of those miles have been pulling our 14k 5th wheel or my dual tandem with a Chevy hunting truck on it, around 13k.
