They do work but are kinda weird to drive with. The engine will bog for a few seconds then spring to life. Its like driving a first gen powerstroke with the lag.
I have one on my truck. If it is set well there won't be much lag, it feels more like the old four barrel carburetors when the vacuum secondaries kick in for that extra boost of power. If you don't have excessive smoke you probably don't need it, though I think it is required in California for 1993-94 7.3's to be technically legal. If your truck does smoke it will help build a little boost before going full fuel.
