81= 81A= base model truck...a number in front of the 81 denotes a base model truck, a letter would denote a Custom Cab trimmed truck
A = 3 speed trans.
12 = DSO..where original selling dealer ordered truck from...Buffalo, N.Y.
You would be correct on the rear end ratio code...12 though is for a 3.89 ratio....if you look under the truck and see a bunch of nuts around the pumpkin on the forward side of rear axle husing, that is a Ford 9" rear end...if not its a Dana 44 if Im not mistaken...
BTW...there oughta be some other numbers after the 12 where it says DSO...if blank thats odd as I am pretty certain 64 models did show the DSO code on the door tags...

The same Warranty Plate was used in 1965.
Besides the DSO, the OP also omitted the GVWR, HP & RPM.
If Limited Slip was ordered, the Spicer-Dana 44 was installed.
The Spicer-Dana 60 with or without Limited Slip was an option.
AXLE code 12 = Used 1961/64 only for this: Ford 9 inch / 3.89-1 / non Limited Slip / 3300 lb. Rear Axle Capacity.
12 was not used in 1965.
12 = 1966: Ford 9 inch / 4.11-1 / non Limited Slip / 3300 lb. Rear Axle Capacity.
