I have done this myself by getting two saw horses, running 2 4x4 (pry the cab up a little) under the cab to the saw horses on one side and a 4x4 streched across 2 more saw horses on the other. put two bottle jacks on the side with the 4x4 acroos the horses and jack the cab up, pull frame out.
im thinking of some homemade lift made of wood?take the front nose clip off of cab and slide cab off of frame to the side or lift it over the motor and pull the chassy out from under
a cab minus the doors and seat is close to 500 pounds so build accordingly I built a frame/craneway with 6 inch I beam for doing this but I do alot of this kind of stuff so I use it often my first cab swap took 4 guys and some 2x6's
pulling the cab off, if its junk, just unbolt it, roll it back, then off, who cares if you damage it. putting the cab back on, i leaned the cab up against the frame, bottom of the cab kind of on the side of the rail. used a engine hoist with the wheels blocked off and lifted up on the ground side of the cab. picked it up and it slip on. worked ok.
I forgot to say that the cab sits on a frame with about 4 inch lift with 38's under it.I got 4x4's under the cab and easter sunday I'll try to round up some poeple to lift it off and place the cab on a trailer. I sure hope I can get 6 or 8 poeple over because the doors and front nose clip are still on! I hope this works and thanks for the info.
I did it myself with the engine hoist using the seat belt bolt holes and a 2X4 bolted across the bottom hinges. It was very top heavy and unstable. Next time I'll try the five guy beer method and leave the front on.
you may want to remove all the glass and save it before you remove the cab it will cutdown on some weight and you just may need or be able to sell the glass

Yeah that's the way i did the bed by myself.
In my case, used 55 gallon drums etc then drove out from under it. :)
I used my green machine to hold up one side and engine hoist and high lift jack on the other side. worked very good! can't wait to put cab on the green machine
