'Blown' engine/failure cause

Recently there are a few of the HO engines that failed. My take is that a few of the new engines weren't assembled properly, maybe a piston coolers failed, fell out, whatever. Seems the common theme with them is a failed piston. If it happens to be the same cylinder on each engine, then that could mean a manufacturing defect in the block. Usually a piston failure is lack of oil, rings that didn't seat, or an out of round cylinder and/or piston. If this is only happening on the HO line, makes me wonder what they are doing different.
