It looks to me like it is either the wrong shock, which is why the coupling nut was used or a correct shock installed incorrectly. Bottom line, you simply need a replacement shock which is correct for the truck. It will come with the required hardware. Best to get a pair, particularly if the other is mounted in the same way.
