I'll be driving my truck down the road and all will be fine, then suddenly misses, steady for 2-3min, then it goes away. Mean while I'm monitoring everything and nothing seems strange. So I decide to do a buzz test, thinking an injector is bad. The code list I get back is crazy, P0261-P0282, every cylinder says injector circuit low. Mean while when this happens my FICM shows a solid 48.5v. I repaired my FICM 10 years ago and it has been flawless since. I checked the connectiosn to make sure they were snug, and all was good. I don't see how it's possible that every single injector has a low voltage, it would have to be somewhere else. I'm not aware of anything between the FICM and the injectors though.
Sounds like your logic side may have taken a dump. I had a customer a few years ago that the injectors would occasionally buzz KOEO. Power side showed a steady 48-49 volts but his logic side was toast. Another shop replaced the Ficm and he was back on the road.
You may also check the wiring harness under the Ficm itself. On some 2004 trucks the rear of the plug may be exposed causing chaffing. It was a common thing on the 2004 trucks. They ended up rerouting the engine harness that year because of the chaffing.
