There are two "notches" (holes) in the column, close to the edge. The square part of the bolt head "catches" the edge of these holes. When you tighten the nuts, it pulls everything together.

Getting the bolt heads to lock onto those holes is probably the most frustrating part of working on these steering columns. It's best to relax and take your time.
I somehow got them to fit into those holes by putting the nuts on about a thread and a half deep and angling them as best I could while pushing the flange on. Took about 3 tries, no pictures because it took both hands. Truck will not start now, but the starter has been on its way out, last few times it has sounded like a dying cat at startup.
