Why wasn't the contamination caught in the two filters before it got to the engine and screw things up?
Hoping that's not a stupid question

The rust in the high pressure system is not introduced - it FORMS on the metal inside of the system when it is exposed to water suspended within the fuel. The issue with the filtration system is that it cannot adequately cause the smallest water droplets to coalesce and be filtered out. The primary fuel filter does have a coalescent barrier but it is not enough some times. And it bears mentioning that DEF will attack the metal inside of the system much harder and much faster than plain water. That is why even the smallest amount of DEF accidentally poured into the fuel tank MUST be removed before starting the engine. THERE IS NO SAFE AMOUNT OF DEF.

Actually an excellent question.
