If the voltages are good with a fresh relay, then the FICM is fine. It's doubtful that this your problem here, but worth a shot. We recommend fresh relays every 100k. Also, just because your alternator is/was charging, does not mean that it is 'good'. I'd recommend pulling it and getting it bench tested. If it's shot, consider an upgraded replacement.
"Other" reading was FVP? FLP? Maybe the relay is crapping out on you.
Thank you Rusty, ordered the relay popped it in, truck started right up, runs awesome and all my FICM readings are great. The relay actually had some melted plastic around the terminals. Anyone know if that underhood relay box has a cover and where I can get one, my relays are out in the open.

It has a cover for sure. Any salvage yards around you?
For now at the least stick a baggie or something over it, keep the dust out. Maybe pop a few small holes in it.
