So here goes. I have a '96 F-250 CCSB 460 4x4 Auto.
Ever since I bought the truck, the trans will go into limp mode with a code 628 and no other codes.
I have been driving the truck around for a while and have made some observations. When I reset the codes, I have noticed it only goes into limp mode when I am in OD going up an incline and give a bit of throttle. When I reset and drive around town or on flat roads, it will not go into limp mode. But the first time I hook up a trailer and go out of town, it goes into limp mode.
I have replaced the filter and done a complete fluid change. When I dropped the trans pan, it was not horribly dirty, just some very minor ferrous dust (from the clutches i presume). I checked all connections, took the dash apart and checked the PSOM board (no problems found).
Am I due for a trans? Is the Shift Solenoid Pack bad? Is the TC bad?
For now I just reset it every time, but it gets annoying. I am just wanting to get a better feel for what I may be in for so I can set aside funds.

I have fixed two of these E4OD trans behind 460's with nothing more than replacing TPS on throttle body. Symptoms weren't exactly same as yours. Pull codes and see what comes up if anything. Lack of ck engine light DOESN'T mean there aren't any codes stored.
Remove the inspection cover from the bellhousing. Look at the forward face of the torque converter. It's probably discolored. That's from heat from it slipping. A new converter will fix this problem.
