Maybe a seized up idler pulley? I've had a couple seize up on our fleet trucks. Try slacking off the belt and moving it by hand. Give the 2 pulleys on the tensioner a spin while your at it, along with all the others if you don't find the culprit.
I think you'd have known it in a hurry if your turbo wheels/shaft was out of whack like that.

My tensioner showed the slightest sign of coming apart(squealed like a bad bearing). Good place to check, mating surfaces should be square with each other.
