I have a 1992 Ford F150 Custom.
The truck will not come out of 1st gear no matter what gear I shift into.
Does that necessasrily mean I need a new transmission?
If not is it a costly fix?

Does the shifter feel normal? You can remove the shift boot and access the 6-8 bolts that hold the top cover on, then you may be able to see the problem.
Something broken in the transmission
Clutch not disengaging.
If you can verify the clutch works fine then carefully take the shifter out as bashby suggested and have a look at the parts, not sure on the ZF but some of these have a little composite bushing that the shift lever moves in which can get broken and jambed, if that all looks good then you may have a broken shift fork inside the trans which means the whole trans has got to come out and get disassembled.
