Ouch, that far apart and it's still making the noise!

I can't see the front of the motor in those images (it's my problem, not your images), is the front off?? (front cover and timing gears)

I've seen some pretty beat up Diesel pistons run for hundreds of thousands of miles!
However, If you can do it right, do it, especially if the truck is worth it!

-Enjoy

I think if possible pulling it and checking everything and at least replaceing the pistons would be the way to go. since it looks like a valve has come in contact with a piston( this would have happened when it ran away with you) it is very possible it squished a rod bearing, so I would definately want to check the crank and bearings.

Heres a little deal that happened to me once. A guys buick jumped time on the interstate once and he was in a hurry so he had me put a timing set on it there was a bent valve in every cylinder, so now we had to tear it down and as he was now in a bigger hurry I had the machine open up and rebuild his heads on the weekend. when we cranked it now the rods were knocking, it had squished the rod bearings.
Now we had new heads and and timing set but a bad lower end and a lot of money tied up, so no choice but to rebuild it, and he was out of time so he had to fly out and come back after it. I felt sorry for the old guy and gave him every break possible, but this cost way more than it would have to go with a reman engine the first time. If you still have the noise I would suspect the bearings at this point.
