In the middle of doing this myself. I wanted to put a frenched in plate as the surface mounts stuck out too far for my tastes. The one downside is that the frenched in allows for four switches, whereas the flush mount allows up to nine. Here are some pics.
I did not like the way that the controller bolted and hung down from my 350, so I made it more "flush" against the lower dash.
And this is the dash. Radio is partially out as I had to fix the CD shuffle.
Here are the switches. These are only temporary, I have the correct translucent green Carlings coming in for the truck. They are being custom engraved.
And the overhead pod. The other three gauges will be in soon.
I am in the middle of a custom switch plate that will replace the lower coat hook. This will also house two gauges. Then I will send the upper dash bezel, lower dash, cup holder, and pod out to be custom printed in wood grain. Obviously I am not finished with the dash job just yet, but should look decent when completed.
I kinda helped popularize this mod a couple of years ago, so here is a pic of mine when I was finishing it up. The horizontal switch is wired into two sensors for the water temp guage. It is an on/off/on switch, one side is water temp, and the other side is oil temp.
Oh, and my brake controller is now mounted up under the 650 dash
I enlarged the original rectangular opening and then made a false back, and filled in the gaps with body filler and sanded, and sanded, and sanded, until I got a level surface under my carlings, primer, and then interior paint

Here is a pic of the new Carlings I have in mine. Love them! I wanted to go the flush mount route for appearance reasons, so I just fabricated my own plate. Not too hard to do.
Also got my gauges in... just waiting on the coolant temp gauge now. The gauges are DiPricol, which is Indian for "Never ships". Eventually, I will be putting two more gauges in the dash where the switches are at and put the switches on a custom plate on the flat spot just to the driver side of the opening (where the air brake control hole is).
ordered a blank one from ed today! cant wait to get it, i will paint it and put my gauges in it! woo hoo.
dang it! looks like shipping is "UPS ground to Alaska is $38.36 , USPS $47.08" through ed! i guess i wont be getting my lower dash it seems! anyone can beat the price on the shipping?

what i dont understand is eds blank dash is 38$ and Clays cheapest one is 86$ ? is 86$ for blank dash too?
No, that is for the modded dash. Ed's price is the same as mine I think on the bare dashes. By the time you box these things up they are pretty big, thats why the shipping is so much. You might message and and see what the exact cost is, some times the website is off.
