Is your engine stock or nearly stock? Is it the original motor and does it have high miles?
If yes to all of these questions I recommend staying with the stock manifolds. Its not worth breaking off the bolts in the heads.
Also, Single exhaust flows more than adequately on our trucks. Just run 3 inch pipe for the single. If you want duals then get a single in dual out muffler and run 2.25 inch tailpipes with a pair of slightly larger tips.
If you need a new Y pipe, there are a couple of prefab choices. Bassani makes one and so does Magnaflow. I also heard that Summit sells a house brand one.
when i was under my truck few days ago. i looked at the flange where the header bolts to the mid pipe and like a chunk of the floange is missing. If i disturbe it im certain it is gonna leake. so thats why i was considering headers. and its a 1996 5.0 4x4 with 98,000 miles. all stock besides for a cat back split dual exhaust
I used headman long tubes on my 95 4x4 and they worked quite well. There was a little trouble fitting them in on the passenger side. They rubbed the frame a little bit, so my friend that was helping with the install, acutally cut out a little piece of my crossmember. Other than that, I thought they performed well.

I actually agree with TexasGuy about leaving it stock. I had to pull the headers when I had to remove my oil pan, and also had a terrible time removing my starter when it went. Not to mention, the bolts in the head were a real PITA. If your going to do it, start soaking those bolts a week in advance with penetrating oil.
A couple of guys have used the pacesetter longtube headers with good results. They are on the top of my list. A bonus is that they are the only ones I've found that come with the egr bung for trucks with the 5.8
