Are your HGs totally gone or is it just puking? Mine started with minor puking that was caused by a clogged EGR cooler. Then it got worse. By using an Exhaust Gas Detector Kit, I was able to detect a little exhaust gas in the coolant tank. That meant it was show time. I just removed the stretched OEM head bolts one at a time and replaced them with studs and did not change the HGs. I also installed the Neal Tech. upgraded EGR cooler while I had the motor apart.
If he has puking and doesn't check the heads to see if they need to be milled or totally replaced he runs a very very high risk of this happening again rather or not he studs the heads. How long it takes for it to happen again, that depends on a lot of factors, but trust me on this, I know from experience that just because you stud the heads doesn't mean that your issues are totally cured.
So I'm going to disagree with anyone who says doing the project with the engine in is too hard.. I did it and it was a piece of cake. Taking the HVAC out is super easy, other than the fact you have to get the system evacuated. Unbolt a few nuts from the inside and the whole unit comes out, fanbox and motor.
As for removing the head bolt on the drivers side, that is an easy job too.. What most people aren't saying is that the head sits on two dowels (holds the head in place before you put the bolts in).. You can put the head on and then lift the back end up about an inch or so, at this point its resting on the front dowell. Once you life it the bolt/stud will side ride in.. This is also true for removing the factory bolt.
I did the heads without a crane, while it was doable it wasn't very fun as its heavy dead weight to move around in such a tiny area.. Sunbelt rentals has them for like 25 bucks a day.. I'd use that again if i were going to do the project over.
Doing the job in cab is intimidating at first, at least it was for me as I've never taken the engine apart. I took my time and learned how everything was put together. It took me a while to take it apart, but only about 10hrs to put it back together.. I'm sure I could rip apart in the same or less.

While I wouldn't say that it would be too hard to do with it in, I would say that there are instances where I wouldn't suggest using that method if there is a need to have something else checked out as well that you are messing with to do the bolt/stud job.
