You don't have to replace anything if you don't want to. I took my cat off and used a crowbar to beat the guts out of it and put it back on. By the seat of my pants, I felt a big difference. By the same token, I put on a 4" exhaust after that and I could not really feel any difference. If you haven't already, you should put in a 3" downpipe. That took me about a half a day.
for a mostly stock truck just gutting it would be sufficient. there is a slight gain from using a straight pipe design instead of the gutted design. having the gutted cat allows the air to become turbulent as it enters the gutted cat and it doesnt flow as well since it is fighting to get out of the cat and back into the tailpipe
I cut mine in front of cat and ran straight 4 " pipe to the rear.The pipe from International dealer(Happens to be my closest big truck shop) was about $3.50 or $4 a foot, Needed about 10'.Open pipe does not seem much louder than stock buy runs much better.I cut it and fabricated/welded a slight angle to bring the pipe out behind the rear tire at a angle.
