I did a search for this code and everthing I found related to it being common when running a chip or tuner. I reprogrammed my truck back to stock about a month ago and now I am getting this code .
When I ran with the scmt on max performance, I never saw this code.
I am also getting p0470 along with the p1211. The truck runs great, I get the code when I get on the pedal to pass or speed up, the SES light comes on and does not go out until I shut down and restart. If I drive it like theres an egg under the accellerator I never get the SES.
I have a 4" dp & exhaust, kwik filter and pillar pod. The boost seems normal runs up to 17 or 18 psi when in stock tune.

P1211 ICP Not Controllable - Pressure Abow/Below Desired
P0470 Exhaust Back Pressure Sensor Circuit Malfunction
There are the codes, P0470 is self-explanatory,
P1211 relates to the Injection Control Pressure. The PCM monitors the high pressure oil system to determine if it needs to be increased if load demand increases. It also uses this to stabilize idle speed. volts in, 1.0volt@580psi, 3.22volts@2520psi. PID: ICP (pressure), ICP V (voltage)

The 1211 code is typically cause by a worn out HPOP. I've read of some trucks getting the code before any chips/tuners. Over time, the effeciency of the pump will begin to wear out. Unfortunately, if it is your pump that's wearing out, there's not much you can do other than replace it.
