The code is in your owners manual on a small card.
if the card is missing, you can take it to your ford dealer and they can run the code for you. i was told dealers would do it at no cost, but you know how that goes. I had to pay $40 to get mine!
Not sure about Superduties but on my Expy it could be found under the dash to the left of the steering wheel, laying head towards the engine. If you get the master code you can use the owners manual to tell you how to set your own code.

I've read of others finding the code there, as well.
Another spot that I've read of people finding it printed is behind the radio.... you can pull it out slightly and look with a mirror/flashlight. Believe it or not, it's printed there.
If you look through your owners manual it will tell you how you can program your own code. I changed the factory code to my own code. If you have trouble most reputable dealers will help you for free.

If its an 2004 or older then its a trip to dealer land, as the keypad is wireless, on the 2005's and newer they are a hardwired keypad and the code is stamped on the security module in the middle lower center portion of the dash. It would take a while to open up the dash to get the number off or the dealer could read it out as well.
Or you could try all 3125 (5^5) different combinations and you should hit it!
I don't have a coded key pad on my F250, but I do on my Expy. The pin is located under the passenger side footwell under the glove box on an electrial box/computer ??? there is a link in a post in the expy forum
