problem with transmission

problem with transmission
when shifting gears in my 93 explorer it is hard to shift gears when going in 1st or shifting in reverse it sometimes grinds going in... i checked everything but can't find the prob .. i changed the tranny fluid and had the clutch changed about 2 years ago thanx for any help in advance
