A broken blend door will flap around and cause intermittent heat then no heat
Pull the actuator and move the blend door manually
I think you'll find yours is broken

If this heat seems to come and go more with the engine speed I'd day it was very likely the heater core has restricted coolant flow through the heater core.

If any sort of "stop leak" or something similar it could be the reason. OP with the engine at full operating temperature feel the heater core inlet and outlet houses near the evaporator housing under the hood, pay attention to how or if their temps rise and fall as the engine is revved up etc.

IMHO if the blend door was the sole fault the temps at the hose ends wouldn't be in sympathy with engine speed.

I have an IR thermometer to trace the hoses to see how hot it is coming into the heater and how much it cools off coming out of the heater... WillyB
