check engine light is gone, driven 500+ miles without it, now: stalls in drive and OD light flashes. I put it in park, restart, no issues. There are no other symptoms I know of.
similar stalling issue a few weeks ago, took it too my shop and they couldn’t figure out what was wrong, there were no codes.
all spark plugs and boots replaced approx 30k miles ago, and the #7 coil only.

The biggest problem was that according to my shop there were no codes, however, I was able to create the failure yesterday: truck in drive and held the brake for about 1 minute as if at stoplight, low and behold! Started to stall, I put it immediately into park and the OD light started flashing and simultaneously check engine light came on.
Friday evening before holiday weekend, no shops open but I did make it to a parts store with a code reader and it read an output shaft sensor failure. I bought the part and replaced it, so far can’t create the problem again. We shall see.
You got very lucky. The parts stores can only read codes that turn on the check engine light. You may have other codes stored that they can't see.
Get Forscan. It is a free download at www.forscan.org. Their page shows how to select an adapter to plug into the OBDII port. I bought mine from Amazon for about $30. If you just read codes it is free to use. If you want to make changes you need to buy a license, and the license is pretty cheap.

You may have 2 problems.
When the engine is running in gear at idle the air intake is controlled by the Idle Air Control valve which bypasses the throttle body and has no OBD codes for poor operation.
A sticky/erratic IAC valve can cause problems with the engine idling properly.
Also, monitor the fuel pressure at the fuel rail test port, I don't think there are any codes for low fuel pressure.
Reseat the PCM connectors, there might be some contact corrosion after 20 years.
The trans OSS OBD code could be a separate issue.
The truck doesn’t seem to have any idle issues except when it’s idle in drive and not moving, so if IAC was faulty wouldn’t it have issues idling in park or neutral as well?
Don’t misunderstand me, I’m hopeful that one of these issues will solve my problem, just trying to wrap my head around if.
I know this is an old thread, but figured I'd throw an update in here. What ended up being the culprit were actually several things, but the primary issue was #9 COP boot had a tear and was likely shorting against the head. Was not getting any codes to that effect either. Once I replaced the whole coil, the shudder and shut down along with flashing OD light went away forever.
In the end, found out I had 2 badly worn valve seats and one bent valve and the head gasket starting to go between cylinder 8 and 9.
