The friction material disc wears down over time, and when it starts to slip, the wear rate is even higher. Most of us catch this with the characteristics the OP mentioned, works at start and less when the load is higher, or variations in output over the drive cycle. While it's dependent on A/C use and freon charge, often it's best to check the gap at some interval, 50k, 100k, whatever it takes. If you catch it when it gets close to the max, often you can prevent excessive wear and a new clutch disc. I think I adjusted mine at 200k.
