I believe it was standard equipment in SC and CC in 79 at least. my 78 Bronco came with it. Was a smart move for Ford, you don't want a customer to freeze.
Could be, but it get's 30 below here in the winter and I've never seen one in a standard cab. but if it cost an extra $2 most people around here wouldn't have ordered it so I guess that doesn't mean much.
I recently sold a 79 Bronco that had that label. Factory A/C truck, had quite a few only one with that label I've seen. Was also a Trailer Special with badge on lower tailgate. I didn't remove heater core but otherwise nothing looked any different.

A huge thank you for the Ford OEM "High Output Heater" label. Received it Tuesday, (29 December 2020) in the mail and installed after lunch today on my dash bezel, centered and directly below the heater controls. Again a huge thank you.
