It is the outside of the stinger (inside of the receiver). If your truck came with a 2.5in receiver, then it should have come with the 2.5 x 2 reducer from the factory (usually under back seat). If you didn’t get, then go back to dealer and get.
They stopped including a reducer with the 2.5" reciever in 2019 MY, only continuing to include a reducer(3"-2.5") with the 3" receiver equipped trucks.

Guess that $1,130 difference gives you the bigger axle and 3" receiver.

^^^. What he said. I had a 2017, 2019 and 2020. The 2017 had 2 reducers 3.0>2.5”>2.0” My 2019 has a 2.5” with no reducers. My 2020 has 3.0” with a 3.0>2.5” reducer.

That explains it. The high capacity trailer tow package gets you the bigger axle and 3" reciever like you mentioned, but also a different spring pack and 10,800 gvwr.
2021 350 has 3" and included 3 to 2.5 sleeve.

Same one I got for one hitch that I use, works fine.
If you get the Ford branded hitch plug (2") it fits snug in the adapter and the plug covers all of the receiver. Looks good.
There's so much slop in my 2.5 receiver that my 2.5 weigh safe hitch looks like this. Going back to B&W for the steel.
I have had aluminum hitches since 2006, several different brands, some adjustable some solid billet aluminum for special applications. Eery one looked like the hitch you posted after some use. Don't recall which one, but one of my adjustables had steel spring loaded ball bearings inset to take up some slop. They eventually failed.

Did you get this hitch?
I got the 3" reciver, 10" drop version, no scale with both double stainless ball options. After a month or so of use pulling a 6000 pound boat I went to change out the ball to the 1 7/8 set. The pin and the ball were seized in the slider.
