Yesterday My truck developed a miss and massive amounts of grayish colored smoke that smelled like raw fuel.When scanned it said #6 contribution.Today I pulled
No.6 injector and the bottom o-ring has a bad place in it..
Wanting to get opinions ,,Bad injector or something as simply as a blown O-ring.
Also for the last year or so The truck has had a "stickion"issue that would go away as soon as truck warmed up.
The issue would never throw a code so until now I wasn't sure which cylinder it was.
My thinking is the injector has gone bad and needs replaced.
Top Pic is No 6 Cylinder
Bottom pic is No 4 Cylinder.
Odd thing is I replaced these O-rings just over a year ago

You definitely had failed copper crush washers at the base from either incorrect torque on the hold down fastener or the mate surface was dirty to start with. Another cause can be if the old copper washer wasn't removed and it had 2 installed on top of each other. Once the copper washer is compromised the compression gases wipe out the lower o-ring on the injector body.
Before installing the hold down make sure you blow out the threaded bore of the fastener. Any oil in that hole can give you a false torque. Clean & inspect the injector bores before installing new injectors. You can use a smart phone camera for inspection.
