Date is November 6th, 1964. Too new to be for a 1964 truck. Looks like this seat came out of a 1965 truck.
The Engineering ID number is a Ford number that also indicates first use in 1965 (C5)

64700 is the base number in the 600 range which is for the seats.
The rest I'm not sure about. The seats were made by vendors that supplied the seats to Ford so it's some information that seems to be unique to each vendor.
