I'm curious... is tuning this difficult for a valid reason - tunes for one flavor of PCM not fully compatible with another? Or did Ford do it this way specifically to keep the tuner industry limited, either to protect IP, warranties, or minimize folks trashing the emissions quality and keep the EPA happy?

It just seems do unnecessarily complicated to me.
In this one particular case between the 1999-2001 and 2002/3 model year pickups, it basically boils down to manufacturers tasking the PCM to control more things. The engine and transmission tuning is virtually identical between these years (and most of them honestly).
