Just an update for those of you wondering if I ever got this figured out, no I did not. I am waiting (still) for Ford to get back to Fairway so they can get back to me. Fairway is certain that I should have remote start via smartlink as I added the factory remote start and enabled it via forscan. Smartlink helpline tells me smartlink will only create a virtual duplicate of the key-fob that came with your vehicle. Nate has said the tier 2 support is slow at getting back to them and wrong 90% of the time. If I don't get a response from Ford by tomorrow, I am returning it all and going with all 12 volt solutions aftermarket stuff.
Got it now and hopefully this will help others. When you are pairing smartlink, the manual says to select the picture of the key-fob that matches yours. Do not do that unless yours has remote start on it. If you installed the factory remote start, you must select the photo of the keyfob that had the factory remote start to enable the feature.

So from reading what you wrote, are you saying that your dealer finally was able to get everything working for you?

Yes - Also let Nate at Fairway know the different step that needs to be taken if you did not have the factory remote start when you purchased the vehicle. If followed correctly it should be 10 minutes to install the remote start and activate via forscan and 15 min at dealer to activate smartlink. This was honestly a miss on Ford as they haven't trained or even made their dealers aware of it.

Complete Junk- After dealer re-programming, remote start button is active on the app, but doesn't work. Returning it all and ordered the aftermarket setup from 12V solutions. Don't waste your time. This is a fail on ford's part.
There is nothing thrilling about mine, but it does everything it’s supposed to do. Flawlessly. And took 10 minutes to set up. Your experience is very confusing to me on all levels.

Me too. The frustrating part is that it has to be setup with IDS and they (ford / smartlink) keep having me reset it and go back to the dealer for programming and its starting to add up. I had a 2018 XLT that did not have factory remote start, but was told this could work. Here is the order of events.
1) Installed Factory Remote start (5 min)
2) Activated via forscan (5 min)
3) Paired Smartlink with keyfob to generate 48 digit key. Selected my factory remote when pairing which did not have start.
4) Brought it to a dealer to add keyfob code via IDS (3 days) - Dealer couldn't figure it out and had to reset the entire unit ($0 charge, but lots of aggravation)
5) Repeated pairing code (Same as step 3) and went to another dealer. In and out in 20 min. Unit paired successfully. Remote start option is grayed out and does not work. ($53)
6) Call Forscan and they reset the unit. Talked to Nate at fairway and he said he installed one in an XLT after installing the remote start, but selected the same factory keyfob for paring except with the remote start icon.
7) Repeated paring step for new 48 digit code except chose the keyfob with remote start. Everything seems to look good as App now has the option active. Just need to have the dealer add the new 48 digit code as it generated a different one.
8) In and out at dealer for programming on IDS. Nothing worked after. Dealer walked me through and showed me he was putting in the key code correctly.($53)
9) Called Forscan and they told me it will only activate the features that your truck came with over the app.
10) Returned to Nate (-$479)
11) Ordered 12 volt solutions remote start and mobile option ($429)
Since upgrading to IOS 12, mine has been unable to connect. Reading through comments on the app, it seems there are problems with it and IOS 12. Lovely.
