I decided to start with an oil change since I had no idea what type or weight oil the previous owner used.
The used oil had just under 5000 miles on it and was black. Put Rotella T6 5w-40 in and it already sounds different with just a quick trip down the road.
Also bought some Cetane booster by Diesel Specialties to put in the fuel tank.
Just because I am overly opinionated and extremely hard headed doesn't mean I know it all. The only thing I will say is that after 9 years of tuning, I have come to realize that these trucks should have come with 80% nozzles from the factory and everyone would have been a lot happier.
Does this mean that a stock truck and injector would benefit from an 80% nozzle?
AD/80% will run without tunes on a 99-01 pretty easily. There is a little more accelerator pedal sensitivity off-idle, but it's not horrible. The 02-03 trucks are a bit less accepting only because Ford fell into the "more sensitive accelerator pedal makes it feel more powerful to compete with the Duramax" school of thought.
The con, however, is that it's like driving a turned-up mechanically injected Cummins. It's going to smoke a bit more until boost comes up and there's no way to reduce it without tuning.

And my next question...my not so 'E'99 has a live tuned PCM, I have been told it performs like an 85 hp tune. Would it need retuning to benefit from that, or would it still be as you have described?

I think the simplest way to describe 80% nozzles on stock injectors is that the injectors will have the capability of being completely empty up to 3000 RPM on stock pulse-width. If your PCM has been programmed with an 85 horsepower calibration, then you will be long out of fuel before the commanded pulse width is over and done. It will also be a smoky mother.
Hooked up FORScan and ran a buzz test.
My #2 is the culprit as it was almost silent compared to the rest.
I can't afford the $1000+ to get them rebuilt with upgraded 80% nozzles.
Looks like I will just have the one injector replaced for now.
I still have PCM woes that have to be dealt with.
Tried running the KOER tests and only the switch test would run. Glow plug and Cylinder tests came back with low voltage errors. Checked alternator output and it was 14.41 volts while idling. Ugh!
