What are some things that will cause a lopey idle on cold start? If it sits overnight when its cold out (below 40F) - the next morning, it will crank over normally, and fire up with a VERY lopey idle. The lope will last about 10-15 seconds then idle normally. The idle doesn't 'smooth out', it just goes straight to normal idle.
I don't have a GP LED, but I can watch the voltage change (Aeroforce) when the relay disengages. And I wouldn't suspect the GPR because it fires right up - just doesn't idle smoothly.
Sounds like Romps. Can be caused by bad glow plugs or thick oil. Have you ohmed out your glow plugs, and checked to see GPR operation? Synthetic oil may help.
Could be a glow plug issue. The glow plug relays go bad often. I installed a led light in my gage pod to be able to see if voltage is coming out of the relay, it doesn't tell you if the glow plugs are working only the relay. At least you will know if your relay fails.

I replaced the GPR with the AIH relay and it still romped. I then changed to 5/40 Delo synthetic and it hasn't romped once.

Thanks for posting the fix, had the same issue with persistent romps at medium temperatures and changing to 5-40 (Rotella though) cured mine as well.
Got close to twice the miles you do and old injectors are apparently a contributor.
How do I explain this to my wife???

15-40 Rotella. Same as last year, with No Romps.

HA!! Tell her its normal on cold starts to have romps unless the sticks are lubricated correctly. Then it all smooths out
I switched to Rotella T6 full synthetic and haven't had the romps since. My truck just went past 290K with the original sticks and still runs good. Complains a little if I start it without it being plugged in below 10 degrees but otherwise, no romps and runs smooth.
