I have no problem with 2-Strokes running backward, but water pumps and secondary CV clutches would probably not work as well...

Secondary clutch works just fine for moving the sled in reverse. You don't run the sled in reverse for very long.

As I recall, you could run them indefinitely in reverse as they also reversed the blower and pump drives. The engine itself had no predetermined rotation unless they were put in tandem. In those cases they ran the engines to counter-rotate, thereby eliminating torque steering.
These were in submarines, river boats and tug boats mostly... Intake air was typically not filtered by anything other than a centrifugal air cleaner that would pull out the big stuff (not much of that out at sea)...
