I just went through this with my 2012 F250. I ended up unbolting the pivot ( part # 4L3Z-83430B39-AA) from the bedside and heating it with a torch. After heating and a few good whacks, it came off. Now I can't find the bushing. Dorman 38641 is supposed to fit but it doesn't look quite right and multiple people have posted reviews saying it doesn't fit 2011-2016 Super Duty trucks.
...and as far as it having a lock that requires lifting the tailgate almost vertical to slide off, that's not the case on my truck. It does have a torsion spring lift assist that put tension on it when lowered but that wasn't what was holding me up. Even with the hinge removed from the truck and no tension on it, it still took heat and pounding to break the rust loose.
This is what the driver's side hinge looks like...or did look like before it was rusted into the tailgate lift assist spring.
