If someone thinks there's a diamond under the crusted cow****, that's their conclusion, not the one the plain language of the ad supports.

I also think there's more wrong than the tranny, however he is being honest. $5500 not bad depending on your abilities and what you want to use the truck for. I have also delt with imports from Canada and they can be a pain in the *** and expensive.
I bought a 2003 with a 2005 engine studed and o ringed heads, new turbo, HPOP, stand pipes and dummy plugs, and new turbo for $3000. Ran bad, owner thought it needed injectors, ended up needing a cam position sensor and then ran great. I put another $3000 in parts, complete new brakes, oil cooler, water pump, and lots of small stuff. I sold it for $6,600. It did have 330,000 miles but looked good and ran great.
It looks like a decent deal to me. If the engine work was done correctly the new turbo and oil cooler are pluses and he's leaving $3,500 on the bone to fix or replace the transmission. If you can save labor costs by wrenching on it yourself, I think there's enough there to stay right side up on cost vs value. To be honest I'm not seeing a lot of rust compared to some trucks and the dings are minor. It does appear to be a basic truck with very few options, but the crew cab and 4x4 do add value. With that mileage it also has a lot of life left in it, assuming it's not one of those turd trucks that seem determined to die. That's really the major gamble.
