I was up on PikesPeak (10k-14.5k feet) a few months ago for the races. During practice, when I would be atleast 12k+ feet and sit there off for 3-5 hours, after starting the truck, it sounded like a piston was going to bust through the engine when under power. During idle/engine breaking (downhill, low gear) it was fine. And once I got to a lower elevation, it completely disappeared.
I am guessing the piston sound is an injector not working. But what does it mean that it only happens at elevation? There is air stuck in there? Leak? Weak injector?
I live in Denver, and drive at 12k' frequently, and I can tell you that it is not a normal thing. You see a lot of black smoke at elevation, but I have never had those kind of problems.

I know it definitely isn't normal. But it was interesting that 1. It only happened after sitting for a bit (making me think there is an air leak that wasn't clearing at elevation) and two, I don't hear it when I am idling/engine breaking downhill. Only when I push on the throttle.
