Unfortunately it can not be flushed, once pluged they require replacement. The coolant passages are so small once clogged even a flush agent will not pass thru to clean it. Are you getting 250 oil temps cruising or towing can you explain the symptoms. How many miles are on the truck, what kind of Maintance has been done. If the coolant has not been serviced properly, it degrades and can lead to a clogged oil cooler.
Normal driving you should not see more then 8-10 degrees diffrence between coolant and oil temp.
Is there really nothing to clean these out with?

2008 6.4 87.000 miles, 1600 rpm crusing goes to 220F at 2.000 240F Towing 17.000 LB trailer pushing 60 mph 265F reduce power mode water at 197F down to 1500RPM 5 min 240F oil. IDLING ALMOST EVEN TEMP??????

Once you remove it I'm sure it could be cleaned in a ultrasonic tank, but with the labor and cost to get it out i don't think its worth messing with best to just replace it.
Here is link to a view of the oil cooler cut open, as you can see the passages are very small. The photo is for a 6.0 but the 6.4 is very similar

Sounds like your on the right path, a bad oil cooler could cause this issue. Running empty you shouldn't see more then a 10 degree delta difference. I would get it looked at ASAP, also i would have the oil changed if you haven't already and still driving it after hitting 265. I wouldn't recommend towing trailers for the time being until the issue is resolved. If you continue to run over 240 you can expect to have engine damage at some point.
