If you look at his picture the truck looks to be lifted, and wider tires. two things that kill mileage. The other major factor is gearing. I am tuned and deleted and have a lift and mud tires on my truck and i get 12-13 towing and 15 empty highway cruising. so you're not out of line. As long as you are deleted which you said you are then the muffler isnt affecting anything. it is just there to muffle a bit but it does not restrict the flow of exhaust or anything.
the best way for you to get better mileage is to keep your foot off of the skinny pedal.

I had 37x12s
6" lift
and got 20-21 at 70 mph
and ran a 12.2 1/4 mile
Also, ANY type of muffler is restrictive to flow to some degree.
My truck has a leveling kit of 1.5 inch lift and stock tires , the pic you saw was of my old 7.3 but I am putting 37 or 38s on my truck eventually with a 6 inch
I have 5" exhaust on my 08. If you look inside the muffler it's 5" straight through with internal louvers in a spiral pattern I guess to baffle some of the noise. The only benefit I see to going straight pipe to using a good flowing aftermarket muffler is cost. I have an H&S dpf and cat delete egr delete Elite intake elbow, cold side pipe, Volant cold air (junk), I also see 20-21 mpg unloaded on the interstate running 65-70mph. Over 70 it's like hitting a wall and mpg drops like a rock.
Well I'll be driving city a lot more than interstate. I think what I'm going to do is go with a 5" exhaust system straight piped probably flo pro and then I'll pretty much hope my mpg goes and be a lot more easier on the skinny pedal
