Check for any updates on your app in case your phone doesn't do so automatically. I'm running Pandora version 5.1.1 on a Galaxy S4. The BLUETOOTH FOR AUTOMOTIVE option should be available under the Advanced menu option under Settings.

I have version 5.2.1 and only have HIGHER QUALITY AUDIO and CONSERVE BATTERY buttons on an Iphone 5

That must be an Android/iOS difference. This is a screenshot of the Advanced menu options from my S4:
If that option isn't available through Pandora, the play hesitation following a call is likely a default Bluetooth behavior on your iPhone. You could try connecting an alternative phone to SYNC and testing for the same result though.
