Nut embedded into the piston
I'm starting to think the guy that worked on my truck was not a certified diesel mechanic. He did not service nor remove and inspect the oil cooler during this repair. I printed of the service bulletin from Ford explaining this as further evidence of a neglectful repair. Once this is all settled, I'll post other details. Still waiting on the attorney.
shop knows it. it probably isn't their first rodeo with stuff like this.
their insistence that it's a valve keeper when it isn't is just their
way of claiming something they didn't work on caused the issue.
that is why they are broken recording you on this.
it looks like the nut was split with a bolt splitter to get it off a stud.
they look like this, and are commonly used to get off a rounded nut.

so, it's likely they split one, and lost half of it into the intake.
