Did you check the tailgate connection? Might have worked loose there and thus the lack of signal.
Yes I did check, actually had my wife sit in truck and watch monitor while playing with wire. The dealership told me they replaced that harness on another truck like mine, the guy was back in a week with same problem.

That is not a good sign. I have been reading a lot of complains re ford electrical systems as of late. Our fleet has been plagued with them however I was hoping that was limited to chassis cab trucks.
It's likely a bad connection due to some corrosion, someplace in the wiring harness. The dealership likely took the easy way out and assumed that it was in the harness closest to the camera. But if you're willing and able to chase the wires from start to finish, you will likely find a corrosion issue (bad contact) in one of the connectors along the wiring. I know it's not what you want to hear, but when I was chasing an intermittent issue with reverse sensors on my last truck. I found a bad connection in the harness up near the transmission.
FWIW, dealers often wont investigate very carefully on some intermittent issues. They know that we aren't going to be happy about paying for 2 hours labor for something they didn't end up figuring out.
Personally, I wish that Ford gave us the option to disable the camera and many other features, all together.
