The only way to know for sure is to install at least a temporay fuel pressure gauge and scanner so you can watch both. There was an excellent article posted here a few years ago about how it's almost never the HPOP. I have the same issue in my truck. One thing I notice is longer cranking during hot starts indicating injector orings leaking.

I am having a real similiar issue in my early 99 seems like a constant 70ish 2000rpm will trigger the ses either let off the throttle and it will go out after a few seconds or stomp on it and get the rpms up 2500-3000 and it will go out after a few. I am getting ready to post a pic of my codes that it threw since last night when I cleared them.
