Tailgate cable replacement

Howdy all!
After reading several posts on the forum, it looks as though my '81 has a newer tailgate OR sombody has changed the hinged brackets for cables. Either way, one of the cables is broken and has been removed.



The other cable does not have the original hardware, so I'm reluctant to use it as a guide.

Question 1:
The bolt in the picture above screws out quite a ways, becomes loose, but won't come out. Do I just need to grab it with pliers and tug?

Question 2:
will I use all of this hardware?



What order does it go together in? Like I said, the one cable that is there does not have the original bolt attaching the cable to the latch. So, I'm reluctant to use it as a guide.

The metal cup lets you tighten the bolt all the way tight, but still lets the cable flop around as it needs to when the tailgate opens and shuts. The plastic washer is just to keep it from rattling around while you are driving down the road.
