I bet its probably either your GPR or glow plugs. I attached a link to help u troubleshoot. Does it crank fine when plugged in?
If you have a lot of miles on your injectors - like 180K or more (depending on frequency of oil changes), your injectors may be getting tired. Switching to 5-40 can extend the life of the injectors, but if you are truely to this point... start planning for an injector swap within about 20K-40K miles.
The glow plug system may be failing, get a meter on your batteries or in your cigarette lighter. The reading while sitting should be just over 12 volts. When you first turn the key, the glow plugs should be active, drawing a lot of current and giving a drop of about a volt (over 11 volts). After the truck is running, the voltage will rise a bit for up to a couple of minutes, then jump to 14.2 all of a sudden. This is usually a good indicator that the GPR system is working, but that doesn't promise all 8 are working.
