The first place to check is the connector under the tailgate. If it wasn't latched properly it could wiggle loose. The camera has it's own 2A fuse also, F73 in the underhood BJB. There isn't much to the wiring:
There are some seven connectors between the camera and the mirror, all potential problem spots.
The video signal is an ordinary NTSC composite signal the same as you get out of any inexpensive surveilance camera or the video out of many gadgets.
You can connect any such source to the truck side of the connector at the tailgate, put the truck in reverse and you will see the video in the mirror if the wiring is all good from the connector back to the mirror.
If you tow campers or other enclosed trailers, you can buy the cable that goes from the camera module in the tailgate to the under tailgate connector for about $50, cut the camera end off the cable and wire the remaining portion that has the connector that mates under the tailgate to a backup camera on your trailer.
You need to use a specific backup camera to get the reversed video you expect, but such cameras are available as cheap as $30 or so. You can also buy another of the nice Ford cameras which also provide the guide lines, and then you just cut that cable in the middle and extend it as needed for the trailer.
Camera on my 2012 F-150 just started acting up. Went to Ford dealership and they said it needed to be changed. Have extended warranty with $100.00 deductible. There is no way to change the camera without changing the Ford logo, because it is built into the logo. Had to purchase the logo for $48.38 plus tax and shop supplies. Owned more Ford vehicles than I can remember. This just isn't right to have to pay to advertise the Ford logo.

Welcome to the forum, Seaeagle1. Just to clarify, did you proceed with having the logo replaced? How many miles are on your truck?
