I have a '96 F250 4x4, 5.8L w/ an E4OD.
The trans had been slipping pretty badly, so I changed the fluid, improved things for awhile. Today, I think it finally pooped out on me...
Is another e4od my only/best option or are there other transmissions available that will be a direct swap?

I don't drive the truck daily, it's used for occasional hauling and plowing.
Check for codes before tossing in the towel or spending more money. These transmissions are completely electronic controlled. You can have a code without turning on the Check Engine Light (CEL).

There is really no other trans that will direct swap.... If you go to a C6 you will loose overdrive and have computer problems unless you change out the computer for one for a manual.. The EOD is not a robust (Ford never put them behind a 351 for a reason) as the E4OD and it would also require a computer change..
So unless you are wanting to convert to a manual (still requires a computer change) the E4OD is going to be your best option... and least work and worries!!!

The grinding, groaning and inability to reverse lead me to believe it's a pretty serious mechanical failure.

Now that you have added more details to your situation, I would agree a new/rebuilt trans. may be in order.
There have been more than a few posts where someone has replace/rebuilt an E4OD to find out they have the same problem as before. I cannot read your mind or listen to the problem from the internet. Thanks for giving us more details. Your initial problem statement of "slipping" did not indicate there were other noises/symptoms with your issue.

I appreciate your advice (didn't intend to come off like a wise *** with my prior comment) while I have your attention is there a specific range the E4OD that will fit my specific Model year and configuration truck? From what I've read so far, those trans from diesels or big blocks will not work. How do I determine what a particular trans was attached to?
Your best bet is a 95+ E4OD from F250/F350 series 4x4. You are correct, the diesel and 7.5L version will not fit. You can use a 4.9/5.0/5.8L version from that era. You could use an earlier version (89-94) if you swap the solenoid block and MLPS from your existing trans. But be aware Ford made internal, as well as electrical, changes along the way. You want the 95+ version if at all possible.
